The reason for the scars is that she had two injuries (they were slashed during a raid) where the tissues on either side of the injuries healed sepereately, rather than together, leaving spaces that interfered with her eating, so Doki sutured them up. Her fingers (and nails) are the result of being mutated from the Last Lamentation, though she still retained the ability to put on her clothes and feed herself, as well as do her hair.
Despite what she has been through, she still retains her cheerful juvenile personality and is smiling. Well, being lucky enough to survive the Lamentation (a series of nuclear wars) would be a reason to smile. Yes, she can see out of both eyes but functions better that the other. She also likes to play in the radioactive snow and is not burned when she does
