He is always very aggressive, when he manages to get an opportunity to relax, he doesn't want to leave it behind. I think Kokushibo is a source of calm for him, so he often clings to him or his possessions when he's alone. Personally I think Muzan clings to Kokushibo out of habit from when he was a lump of flesh, so the sight and scent of him brings a feeling of security.
